Submitting Your Hospital Admission
All patients, public and private, are required to submit a hospital admission form ahead of their procedure. We prefer you submit your hospital paperwork, using the links below. However, if you wish to complete a paper form you can collect this from our reception. Once you have created an Account you will be directed to complete a new Admission for your surgery. Please answer all necessary questions to the best of your ability. Once you have finalised your Admission an email will be sent to you confirming your Admission has been processed and will contact you directly one business day before your procedure to advise admission time.
