All patients, public and private, are required to submit a hospital admission form ahead of their procedure.  

 

Patients will receive a hard copy admission form (RFA) with the rest of their paperwork from our front
desk after their consultation with Dr. Vasica, however this can also be done online.

Once you have completed your admission, the Hospital will send you a confirmation email and will
contact you directly one business day before your procedure to advise admission time.

Please refer to our Pre/Post Operative instructions to ensure that you are across any preparation
required for your surgery including medication that should be ceased.
